Medical Record Technician (JD-0057)
Overview: The Medical Record Technician (MRT) plays a critical role in the hospital by organizing, managing, and maintaining patient health records. The MRT ensures that patient files are accurate, updated, and confidential, supporting efficient clinical care, legal compliance, and health information reporting.
Key Responsibilities & Tasks
- To prepare file, retrieve, and organize patient records (both paper-based and electronic).
- Ensure all documents (lab reports, diagnostic results, discharge summaries, etc.) are properly compiled and indexed in the patient’s file.
- Assign and track Medical Record Numbers (MRNs) and manage patient registration data.
- Accurately enter patient data into the Hospital Information System (HIS) or Electronic Medical Records (EMR) systems.
- Verify completeness and accuracy of patient information and correct discrepancies.
- Maintain the confidentiality of all patient records in compliance with HIPAA or local data protection regulations.
- Assist in the secure release of medical records to authorized personnel or institutions following hospital policies.
- Assist in coding diseases and procedures using ICD, CPT codes under the supervision of a medical coder or officer.
- Flag incomplete, missing, or inaccurate entries for review and correction.
- Support audits and ensure documentation is ready for quality assurance checks or accreditation purposes as per NABH/ JCI compliance.
- Retrieve and deliver requested patient records to clinicians or authorized staff promptly.
- Maintain proper storage, labeling, and archiving of records as per the hospital’s record retention policy.
- Assist in generating basic health statistics, admission/discharge data, and monthly reports as needed.
Qualifications & Experience:
- Diploma/Certificate/Degree in Medical Record Technology or Health Information Management.
- Minimum 5+years of relevant experience in a hospital or clinical setting preferred.
- Familiarity with Hospital Information Systems (HIS) or Electronic Medical Records.
- Understanding of patient data confidentiality and legal guidelines.
- Good computer skills and typing proficiency.
- Ability to work independently and in a team.
- Effective communication skills (written and verbal).
